How to Make a PERKESO (SOCSO) Claim in 2026: Form 34, Documents and Benefit Rates

Last verified: 31 October 2026. Short answer: PERKESO's Employment Injury Scheme covers work accidents, commuting accidents, emergency accidents and occupational diseases [PERKESO]. To claim, complete Form 34 (Borang 34) with the supporting documents and submit it at the nearest PERKESO office, or create an accident, occupational disease or death notice on the PERKESO Faedah portal [PERKESO]. Temporary Disablement Benefit is paid at 80% of the average assumed daily wage, from RM30 to RM158.67 a day, when medical leave is at least 4 days including the accident day [PERKESO]. Claiming is free: PERKESO advises against paying middlemen or agents [PERKESO checklist].

What the Employment Injury Scheme covers

  • Work accidents while at work or on a business day [PERKESO].
  • Commuting accidents between home and work, on a journey directly connected to the job, or to and from a meal during an authorised break. An accident during a detour or interruption is not covered [PERKESO].
  • Emergency accidents at the employer's premises while helping or rescuing others [PERKESO].
  • Occupational diseases listed in the Fifth Schedule of the Employees' Social Security Act 1969, such as noise-induced hearing loss and occupational asthma [PERKESO].

Step by step: making the claim

  1. Get treatment and keep the medical certificate. PERKESO asks for the original MC or medical certificate for an accident notice [PERKESO checklist].
  2. Make a police report for road accidents. The original report, or a copy certified by the police station that issued it, is required for road accidents [PERKESO checklist].
  3. Download Form 34 (Notice and Benefit Claim Details) and complete only the sections marked for the applicant [PERKESO] [Form 34]. It covers the insured person, the employer, employment details and the accident details, including time, place and witnesses [Form 34].
  4. Gather the documents on the checklist (below).
  5. Submit the form and documents at the nearest PERKESO office counter [PERKESO]. Alternatively, an accident, occupational disease or death notice can be created on the PERKESO Faedah portal [PERKESO].

Documents checklist (accident notice)

Use A4 paper for photocopies and fill in the form in black or blue ink; complete documents help the claim move faster [PERKESO checklist]. Most PERKESO forms are in Bahasa Malaysia only, so ask PERKESO or someone you trust for help if needed [PERKESO].

Worked example (our own arithmetic)

If your average assumed daily wage is RM100, Temporary Disablement Benefit at 80% is RM80 a day. Ten days of certified medical leave would be about RM800, before any days you actually worked.
